Flow Chart for Providers and staff to follow for return to work or not with Covid SX

I was wondering if anyone has an easy to read flow chart for a clinic to use for potential exposure to covid and potential covid -19 symptoms and returning to work?

Like this one, but specifically for HCW in clinics, both Providers and staff?
