For those who are interested, I am happy to share our COVID vaccine waivers for the first and second doses. I mentioned this in the last PMI Webinar.
It’s fairly lengthy and could definitely be shortened. Back in February when we started giving vaccines, we adapted it from several other waivers we saw being used by our local hospitals and health departments.
Happy to help anyone who is embarking on this! We’ve vaccinated over 8,000 people in our practice drive-through COVID vaccine clinic so far and still going strong.
